OTN’s Telehomecare program has now reached more than 7,000 patients in Ontario, and is available in 10 Local Health Integration Networks, with more expected to come on board shortly. An average 50% reduction in hospital admissions and visits to the ER have been reported by program delivery sites, both during the program and six months post discharge.
